Instructions
Business Name-
Enter the name of your company or, if you are an individual, your first and
last names.
Daytime Phone-
Enter a telephone number where you can be reached during business hours.
Street or Postal Address
- Enter your postal mailing address. This
is not an email address.
Fax/Eve Phone-
Enter your fax number or a phone number where you can be reached outside of
business hours.
City, State, Postal Code
- Enter your city, state, and postal (zip) code.
Country-
Enter the country you are in, in English. Leave blank for United States.
Contact Person-
Enter the name of the person who is responsible for this account. If this is
other than yourself, items 2 & 4 should enable us to reach this person.
Current Email Address
- Enter the current email address of the listed contact person. If that
person does not have email, please enter your email address. If you do not
have email, please enter the word "NONE" in all caps.
Retype Email Address
- One common problem is that users mistype their own email address.
To help guard against typographical errors, please retype your email address
a second time in this box, then compare it to box 8a to make sure both are
correct and match on another.
Account Size-
Select the size of the account you need. Most people
overestimate their needs the first time around. If you
use more space than you order now, upgrading is easy. Remember,
92% of all web sites are less than 5MB's. Unless yours is especially large,
a 5MB account will usually be adequate.
If more than 20MB's enter size
- If you require more than 20MB's, how much space do you
require?
If 1-4 MB's, upgrade to
DNS-based VS?- If you need less than 5 MB's of space, would
you like to upgrade to a DNS-based Virtual Server for an additional $5.00
per month? The DNS-based Virtual Server will allow all Web browsers
to access your site directly, using your domain name, whereas the HTTP-based
Virtual Server included with 1-4 MB accounts will only work with the newer

Account Type-
Select Basic if you want only FTP/Telnet/SSH access to your Netmar account.
Select SiteMaster if you also want access via the
SiteMaster Comprehensive Website
Management System.
Desired Username-
Select a 3-8 character alphanumeric username. Please use ALL LOWERCASE
letters and do not include any spaces. The name must begin with a
letter and can contain only letters, numbers, and the dash character ("-").
Desired Domain Name
- Please enter the domain name you would like to use with this
web site. This should be only the second-level domain name. That means that
it should be of the form DOMAIN.COM, or DOMAIN.NET. WWW.DOMAIN.COM is NOT
a second-level domain, nor is john@DOMAIN.COM. Note that you can use a name
you already have registered with the InterNIC if you would like to move it
here to Netmar from your existing provider.
New/Existing-
Specify whether the name listed in Item 11a is an existing name you already
own, or whether this is a name you would like to registed with the
InterNIC for the first time. Note that new registrations are subject to
the InterNIC's $100 2-year prepayment.
Payment
Method- Select the method of payment you would like to use to
setup your account.
Corporate PO
#- Enter your corporate purchase order number. Afterwards,
please fax your purchase order to (919) 220-6061 ATTN: New Orders. If your
company does not have a Purchase Order system, and you do not wish to pay
by credit card, please click here for instructions.
Credit Card
Number- Enter your credit card number. This form is encrypted,
so transmission of your card number via this form is safe. Selecte one
box per column. Each column corresponds to a single digit of your card
number and can be set from 0 to 9.
Name On
Card- Enter the name exactly as it appears on the card.
Credit Card Expiration
Date- Enter the credit card expiration date as four digits.
For example, June 1999, would be 0699.
Credit Card
Type- Select the type of the credit card. MasterCard, Visa,
Amercian Express, Discover, Carte Blanche, and Diner's Club cards are accepted.
Where did you hear about
Netmar?- Please select, from the drop-down menu, how you
heard about us. If the method is not on the list, please select other and
tell us how you heard about us in the next box.
Please specify Usenet
newsgroup, referral, or other.- If the how you heard about
us was not listed in box 17, or if it asks for more specific information,
please enter that information here.
